Job Description

Summary:

The Certified Diabetic Educator (CDE) works as an integral part of the clinical multidisciplinary team. Service focus is on Health Education to patients/clients. CDE is responsible for assessing, organizing, implementing and evaluating health programs related to the care of the person with diabetes and participates in planning and implementation of departmental offerings. Health educator conducts one-on-one health education consultations as well as classes on various health education topics. Assesses patients' educational needs on an individual and/or group basis in collaboration with members of the care team and applies this assessment to the design and delivery of educational interventions. Documents service delivery and patient goals within the electronic health record. CDE provides support and acts as a resource for physicians, healthcare providers, councils/committees as designated, and provides occasional lunch-and-learns for colleagues.

Participates in health fairs or community events on weekends as requested 3-8 times a year.  All other duties as assigned by position's supervisor.

 

Job Knowledge: Must possess proven professional and cultural competence, ability to work independently and to function accountably within a team. Demonstrates highly effective interpersonal, written and verbal communication skills. In-depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns. Must possess knowledge of community services and demonstrate a high level of community collaboration. Experience in group facilitation and motivational interviewing. Basic knowledge of Microsoft Office programs and comfort working with electronic health records:

Education:Certified diabetes educator (CDE) certification required.

ExperiencePrevious experience working in health education and with the target population (uninsured, underinsured, homeless) preferred. Teaching experience and presentation skills required.
