Summary
To prioritize all incoming patient calls utilizing an emergent, urgent, acute, and non-acute level classification system based on information provided by the patient over the telephone. Once prioritized, the triage nurse will solicit additional patient information, answer questions, and, within his or her scope of licensure and education, provide limited health education to the patient. The triage nurse may also schedule the patient an appointment at one of CFHC’s clinic locations, if necessary. This position is responsible for ensuring all calls are documented on required forms and in the Electronic Health Records (EHR). The Telephone Triage Nurse Service is offered as a complement to Coastal Family Health Center’s mission of delivering prompt and high quality primary health care services to all patients. Under no circumstance should it replace the face-to-face relationship that has existed between a patient and his/her healthcare provider.
Knowledge, Skills, and Abilities
  • Must have a thorough knowledge of practical nursing theory.
  • Must possess a working knowledge of standard practices, rules, and regulations related to nursing.
  • Must have the ability to keep accurate records.
  • Knowledge of anatomy and physiology, aseptic technique.
  • Must be competent and have strong organizational skills.
  • Must have the ability to handle stressful situations and to meet assigned deadlines.
  • Must have excellent interpersonal and oral/written communication skills.
  • Screens and responds to patient phone calls, consulting with healthcare providers as needed.
  • Furnishes medical advice per approved triage reference books.
  • Schedules appointments, based on necessity, by offering the earliest appointment available with each provider.
  • Documents all triage actions in the designated Triage Log in clear and precise terms to include time and date of the call, symptoms stated, assessment made, and course of action. Additionally, if caller is a Coastal patient, the same documentation must be entered into the EMR.
  • Initiates appropriate follow up as necessary.
  • Maintains confidentiality of all patient information. Adheres to policies/safety practices established by CFHC.
  • Performs other duties as assigned.
Qualifications/Education Requirements
Graduate from an accredited school of nursing. 2. Possess a current Mississippi license to practice as a Registered Nurse. 3. Two years clinical experience as a Registered Nurse preferred but not mandatory, Current CPR certification required (must be obtained within 30 days of hire), Must be proficient on Practice Management and EHR HIT systems.
