Using funds provided by the Delta Health Alliance, MPHCA facilitated partnerships among hospitals and health centers in the Mississippi Delta to help patients identify a medical home and reduce the volume of non-urgent care delivered in hospital emergency departments. (This program is no longer functioning due to budget cuts.)
