Overview

In collaboration with MPHCA, the National Nurse-Led Care Consortium is convening a learning collaborative of Mississippi CHCs interested in learning more about UDS Mapper capabilities, as well as best practices for engaging community partners in supporting health center programming and patient outcomes.

Timeline & Commitment

Participants in the virtual learning collaborative will attend 4 learning sessions consisting of a pre-recorded webinar, a live demonstration of the UDS Mapper, and 2 peer learning sessions. These learning sessions will be hosted early December 2017 through January 2018 in preparation for completing the 2017 UDS report. Each learning session will be an hour in length.

Outline

Module 1 – Pre-recorded Webinar and Resources

Early December 2017

  • UDS report updates and requirements
    • Location-based count of public housing residents
    • Unique reporting and counting requirements for public housing primary care sites
    • New UDS Mapper capabilities
      • Incorporation of HUD data
      • Utilizing zip code tabulation areas (ZCTAs)
      • Creating reports to assist with UDS completion

Module 2 – Peer Learning Session

Mid-December 2017

  • Peer-to-peer learning session and opportunity for personalized consultation

Module 3 – Live Demonstration of UDS Mapper

January 2018

  • Virtual training opportunity
    • Health centers can join live demonstration of new UDS Mapper capabilities
    • Troubleshoot issues
    • Test capabilities with sample data

Module 4 – Peer Learning Session

February 2018

  • Peer-to-peer learning session and opportunity for personalized consultation
